Bereavement claim form

If you've lost someone close to you recently, you can use this form to let us know. There’s a little bit of preparation to do – you can follow the steps we’ve laid out here.

Step 1. Scan or take a photo of each page of the death certificate and have the deceased’s details to hand. Or look out a copy of the certificate if you had one emailed to you. Email the certificate to us at mailbox.estates@scottishwidows.co.uk. If you prefer, you could post the original copy of the death certificate, or a copy certified by a solicitor to us at Scottish Widows Bereavement Team, PO Box 24177, 69 Morrison Street, Edinburgh, EH3 1HT.

Step 2. Please complete the online form below.

Step 3. Submit the completed form by clicking Submit at the foot of the form so we can get started on your claim. Our bereavement team will be in touch in the next few weeks to let you know what happens next.
